As a new patient, you’ll receive the following paperwork by mail. If you haven’t received it or if you misplace it, you can print these forms and complete them before your visit. If you need help, please reach out and let us know.

What is Inform Me?
- Inform Me helps patients make informed treatment decisions about whether to accept or to refuse a kidney from an increased risk donor.
- Includes four chapters with brief text, videos and graphics
- Survey questions after each chapter
- Focuses on kidneys, but also applies to other organs
